Founded and led by Registered Nurse and Certified Alzheimer’s Disease and Dementia Care Trainer, Diana Nelsen, Evergreen was created to bring the power of professional nursing into the home—where comfort, healing, and dignity belong. Every client receives individualized attention and care designed to meet their physical, emotional, and cognitive needs. From daily assistance to dementia-specific support, our nurse-led approach helps clients live safely and independently in the place they love most.

Diana brings more than twenty-five years of clinical nursing experience and a deep commitment to educating others. Having completed extensive training in Alzheimer’s and dementia care, she provides professional consultations, seminars, and certification courses for individuals, families, and healthcare organizations. Her work blends clinical knowledge, compassion, and innovation—empowering caregivers and professionals to provide care that is informed, respectful, and truly heart-centered.
